Thank you for your interest in becoming an HRA Junior Volunteer Pet Pantry Stock Assistant. 

If you are a high school sophomorejunior, or senior, available to volunteer on various afternoons and/or weekends for two hour shifts helping to package and stock cat and dog food for the Pet Pantry, please complete the following application. The Pet Pantry Stock Assistant is an important role that supports the area pet food distributions. This position does not involve animals, nor are animals at the site.

WAIVER -- Release of Liability
I hereby certify that I am the parent or legal guardian of the minor named above, that I am at least eighteen years of age, and that I consent that the minor named above has permission to participate, if selected, as a PET PANTRY STOCK ASSISTANT in the Humane Rescue Alliance’s (HRA) Junior Volunteer (JV) program for high school SOPHOMORES, JUNIORS, and SENIORS.
I understand that the Pet Pantry Stock Assistant is a supportive role and tasks are completed at HRA's property at 10th and M Streets, S.E. or HRA’s animal shelter located at 71 Oglethorpe Street, N.W., Washington, DC 20011.
I understand and acknowledge my child will NOT be working with animals. The PET PANTRY STOCK ASSISTANT role supports the Pet Pantry by moving and packaging pet food and preparing it for future distribution.
Although there should be no animals on site at any of the prep sessions, I understand that HRA works with living animals that can be unpredictable and potentially dangerous in an unexpected manner. By signing below, my child and I knowingly assume the risk of participating in any activity involving HRA. I hereby waive any and all liability of HRA and its current or former employees from any harm, injury, damage or claim of any kind held by me or my child related to any action or inaction of HRA in conjunction with the activities and/or occurrences of any kind at HRA. Furthermore, my child and I understand that there is to be NO interaction with unavailable/staff only animals during any time spent at HRA or any HRA function.
My child and I understand that possession of any tobacco, alcohol, illegal drugs, firearms, or explosives are strictly forbidden. Participants in possession of tobacco, alcohol, illegal drugs, firearms, or explosives will be expelled from participating in the program and all HRA activities.
I understand that HRA cannot guarantee that my child will not be exposed to or come into contact with nuts, soy, milk or wheat products. Peanut butter is used routinely in animal treats. If my child uses an EpiPen, he/she is trained to administer it.
My child and I understand that the taking and/or sharing of photographs of videos of any unavailable animals or of any nonpublic locations at any of HRAs facilities is strictly forbidden.
Finally, by signing below, I give HRA permission to photograph my child and to post my child’s first name along with his/her photograph and/or writing in any HRA publication and/or HRA web or social media site. By signing below, I grant HRA an irrevocable, non-exclusive, perpetual license in the above-noted photos and/or writings. I further consent that such use and license shall be free and that I hereby waive all rights to compensation.
I am aware that this is a release of liability and a contract between HRA, me, and the MINOR. By signing this form, I agree to the terms of this contract freely and voluntarily.
I understand that by signing and submitting the Release of Liability, I am giving my permission for the applicant named above to participate in HRA’s JV program as a Pet Pantry Prep Stock Assistant should s/he be accepted to the program.
